Personal Statement:
The personal statement should outline the applicant's current or previous work as relevant to plans and objectives for advanced study, describe professional goals and how these were developed, and state the reasons(s) the applicant desires admission to the Pharmacy Systems, Outcomes and Policy graduate program at the University of Illinois at Chicago. The statement should not exceed two pages. Again you will receive an email directing you how to submit the personal statement after you complete the on-line application.
Recommendation Letter:
The Department requires four completed letters of recommendation. Authors of letters for applicants may either use the UIC recommendation forms and/or regular letterhead stationery for typed and signed letters. Note that after you authorize the release of letters of recommendation (in the on-line application) and complete your application, you will then receive instructions by email on how letters of recommendation should be submitted.
